At Aluna Vein Centers, we proudly offer a wide range of affordable cosmetic, laser, and vein treatments and procedures at our four state-of-the-art offices in Beverly Hills, Encino, and Glendale. Whether you’re considering one of our Los Angeles non-surgical cosmetic procedures or need varicose vein treatment, we never want financial roadblocks to stop you from receiving the treatment you need. That’s why we accept most PPOs and Medicare insurances for medically necessary procedures.

About Medically Necessary Procedures

Health insurance companies usually cover treatments and procedures they deem “medically necessary”. Signs and symptoms of venous insufficiency, peripheral vascular disease, ultrasound results, and three to six months of conservative measures are typically enough to qualify for insurance coverage in the diagnostic stages.

Whether your treatment is covered is ultimately dependent upon the type of insurance you have. To help us determine if your treatment will be covered, please have your insurance card readily available while scheduling your appointment.

Prior Authorizations

Many insurance companies require that a “prior authorization” request be sent in and approved before they will cover the treatment. Approval for treatment coverage depends largely on  medical necessary which include doctor complete consultation and ultrasound results, but also on the insurance company’s definition of “medically necessary”.

Once your insurance company has determined that you qualify for coverage, we’ll call you to set up your treatment. Because a prior authorization typically comes with an expiration date, it’s important that you receive the treatment you need while it’s still approved.
Note: Getting approved for treatment coverage doesn’t always mean that you aren’t required to pay anything. You may still have monetary responsibility for certain procedures.

Services Not Covered by Insurance

There are some services that insurance companies won’t cover are all cosmetic procedures including:

  • Spider vein treatment
  • Botox and Dermal Fillers
  • Fat Transfer treatment
  • CoolSculpting (Freezing Fat Removal)
  • All Laser Cosmetic Procedures: Skin firming and lifting, clear skin

Q: What types of insurance plans do you accept?

Q: What should I bring with me to my appointment?

A: We ask that you please bring the following to your appointment:
  • Insurance card
  • Photo ID
  • Any relevant medical records from other health facilities
  • Method of payment for your portion of financial responsibility

Q: Are there forms I can fill out before I arrive at your office?

A: Yes. Please fill out, print, and bring these registration forms with you.

Q: Do you accept credit cards?

A: Yes, we accept payment via the following credit cards:
  • Visa
  • MasterCard
  • American Express
We also accept cash, checks, Care Credit, and money orders. Checks should be made payable to Ramtin Massoudi, MD. Returned checks will be assessed a $25 fee.

Q: Do I have to pay at the time of my appointment?

A: Yes. Any deductibles, co-payments or co-insurance are due at the time of service. The specific amount of your financial responsibility is determined by your health plan as either a fixed dollar amount or a fixed percentage of the contract allowable amount. Please refer to your health plan for specific information on financial responsibility.

Q: What if I’m running late for my appointment?

A: Please call our office as soon as you know you’ll be late. We’ll do our best to accommodate you, but we may have to reschedule your appointment.
