What is an Ambulatory Microphlebectomy?

A phlebectomy is a procedure that removes a varicose vein surgically, through tiny incisions in the skin. Dr. Massoudi will usually opt for an ambulatory microphlebectomy if a vein is to bulgy and twisted for endovenous laser treatment (EVLT). We may also opt for sclerotherapy or Varithena in this case, but in some instances, a phlebectomy is the best option.

About the Ambulatory Microphlebectomy Procedure

Thankfully, this is an in-office, outpatient procedure that requires little preparation or recovery for the patient. We ask all patients to shower before their appointment, as you'll need to keep the bandages clean and dry for 24 hours during the procedure. You may also need to adjust your exercise regimen, as you'll need to refrain from vigorous exercise for two days following the procedure.

The treatment itself is a simple, minimally invasive procedure. While the specifics may vary from patient to patient, treatment goes roughly like this:
  • We may provide you with a mild anti-inflammatory or relaxant medication
  • Your doctor will carefully sterilize the area
  • Your vein doctor will then use a specialized light or ultrasound device to map out your varicose veins on your skin with a marker, making them easy to trace.
  • Once mapping is complete, your doctor will inject a local anesthetic to keep you comfortable during the procedure.
  • Your doctor will then make a series of 2-3mm long incisions along the problematic vein.
  • Using a hook-like instrument, your doctor will carefully remove the vein through the incisions. After closing the incisions, your leg will heal normally and blood flow will divert to nearby veins.
  • After the procedure, we'll carefully bandage your leg and place a prescription compression stocking over the bandage.
  • You'll need to walk for 20 minutes immediately following the procedure, but once that's complete, you're free to go.
  • Unless we give you a sedative, you're free to go about your day normally after the procedure.
  • We'll schedule follow-up appointments 2 weeks and one month after your procedure to ensure the leg is healing normally.
After you leave our office, you'll need to keep the bandages and compression stockings on for 24 hours. After 24 hours are up, you can remove them to take a brief shower – just don't remove the steri stips underneath them, as those will fall off on their own. We recommend wearing your compression stockings during the day for at least 2 months after your treatment, removing them only for hygiene.
While you'll need to avoid strenuous exercise for 48 hours after the procedure, we encourage you to stay active and perform your daily activities as much as possible. If you're dealing with pain, we recommend using Tylenol, Ibuprofen, or Arnica, a homeopathic supplement.
